New Patients
We are located in Claremore on the north side of town on Hwy. 66 (Lynn Riggs Blvd.) and the Stuart Roosa stoplight (one light north of Blue Starr). We are on the west side of the road between State Farm and Animal Medical Center.
To help us better serve you:
We are a specialty office, so we require all new patients to arrive 45 minutes early to complete necessary paperwork. It will take you approximately 30 minutes to complete your paperwork, and the nursing staff will visit with you 10 minutes prior to seeing the physician. Dr. Welch strives to run on schedule to better serve your needs. If you arrive late, we might need to reschedule your appointment.
We are required to keep a current copy of your insurance card(s) and license on file. Please beware if you do not have your cards at the time of service, you will be considered private pay for the visit in full or you may reschedule your appointment.
We are required by law to collect all co-pays at the time of the service. No exceptions can be made.
As of July 7, 2008, we are required a 24-hour advanced notice of all cancellations. There will be a minimum $25 fee charged if notice is not received.
All follow-up appointments MUST arrive 20 minutes prior to your appointment to fill out necessary forms. If you do not arrive 20 minutes prior, your visit may have to be rescheduled.
